The Caregiver is a paraprofessional member of the home care team who works under the supervision of a registered nurse and performs various supportive services as necessary to meet the patient's needs. The Caregiver or otherwise known as Attendant Care Worker is responsible for observing the patient for changes in condition, reporting these observations to supervisory staff and documenting observations; and care performed.

Essential Responsibilities

1. Cleaning
a. Dusting
b. Cleaning bathrooms
c. Cleaning windows (if necessary to attain safe/sanitary living conditions)
d. Cleaning oven and refrigerator (if necessary to prepare food safely)
e. Cleaning kitchen
f. Washing dishes
g. Changing linens
h. Making beds
i. Routine maintenance of household appliances
2. Laundry
a. Washing, drying, and folding laundry
b. Ironing of clothes (only if clothes cannot be worn otherwise)
3. Shopping for food, medicines (case-by-case basis) and household supplies
4. Nutritional Maintenance
a. Planning and preparing meals
b. Assisting with eating
5. Personal Care
a. Assisting with showering, bathing, toweling, dressing, and shampooing
b. Assisting with transfers to / from wheelchair or bed
c. Assisting with toileting, excluding bowel programs
d. Assisting with routine ambulation activities
e. Assisting with fingernail and skin care
f. Assisting with tasks necessary for the comfort and safety of movement-restricted patients
g. Assisting the patient with any special appliances and/or prosthetic devices
h. Train the patient, the patient's family members and/or friends in personal care tasks
6. General Supervision
a. Providing medication reminders and assist with self-medication
b. Monitoring the patient to prevent personal injury and/or property damage
c. Monitoring patient's condition and report changes in a timely manner
d. Maintaining a structured schedule which meets the patient's needs
e. Coordinating with the patient and/or patient's family to medical appointments
f. Coordinating with patient and/or patient's family to obtain other necessary services in order
to meet the objectives of the patient's' care plan
g. Providing companionship suited to patient's ability to participate in shared activities
including conversations, games, reading, outdoor activities, etc.

Position Qualifications
1. At least one-year prior experience.
2. Meets the training requirements in accordance with State and Federal laws.
3. At least 18 years of age.
4. Ability to read and follow written instructions and document care given.
5. Self-directing with the ability to work with little direct supervision.
6. Empathy for the needs of the ill, injured, frail and the impaired
7. Posses and maintain current CPR and First Aid Certification
8. Demonstrates tact, patience and good personal hygiene.
9. Licensed driver with automobile that is insured in accordance with Organization requirements
and is in good working order or otherwise have reliable transportation.
10. Within 45 days of hire, must complete the company provided Direct Care Worker Training
Program.
